Ski club ticket order form, Please turn in Contact Form with ticket order form, if you're first time.

 

Membership: 

 

Forms must be signed:  Medical Release Form 2011-12 

 

Lift Ticket(s) Sold:  Before school on Thursday.                                           

 *Tickets sold on Thurs. night at Mt. Holly beginning at 5:00pm

 

Purchase Ticket(s):  Exact Change, if Check ,make payable to:  Mt. Holly

                                  Please place Money in Envelope. (This makes it easier to disperse tickets)
